Description
- Provides patient care, under the supervision of a Case Manager: Nurse or therapist.
- Provides personal care/companionship as directed by plan of care.
- Provides light housekeeping/ light meal prep tasks as directed by plan of care.
- Effectively communicates both verbally and in writing.
- Initiates and maintains positive relationships with patients/customers and co-workers.
- Observes and reports patient status changes including skin and psychosocial.
- Takes responsibility for self-development and supports a learning environment.
- Displays commitment to the mission of UHHS/UHHCS and its value.
- Performs other duties as assigned.
- Complies with all policies and standards.
- 100% travel as the patients will receive care in their homes.
- Full time 40 hours per week.
- This will be the day/evening rotation.
- The position is in Home Care services.
- For specific duties and responsibilities, refer to documentation provided by the department during orientation.
- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.
- Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
- Full time 40 hours per week.
- This position is in Home Care Services.
- 100% travel as you will be visiting patients in their homes to provide care.
- Day/evening rotation.
