Under the direction of the Ambulatory and Population Health Pharmacy Manager the emphasis of this position is to deliver high value clinical pharmacy care in both an embedded and telehealth model. The focus of efforts will be in providing comprehensive medication management for a primary care population.

Reporting Relationship

The position will report directly to the Ambulatory and Population Health Pharmacy Manager

Essential duties and Responsibilities

The primary job responsibilities include but are not limited to the following:

* Develop collaborative, engaged physician and patient relationships to increase referrals and participation in the clinical pharmacy program

* Take initiative to appropriately optimize and prescribe new medications within a consult agreement or collaborative practice agreement (CPA)

* Ensure adequate patient follow-up and monitoring to improve chronic disease management, quality metrics, medication safety and adherence

* Increase prescription volume to our internal UH retail pharmacies and/or home delivery

* Adapt to evolving prescription benefit management within our health plan to deliver targeted interventions around formulary and utilization management

* Identify new opportunities to advance clinical pharmacy services

* Educate students from schools of pharmacy and pharmacy residents within the UH Health System

* Serve as a medication resource to employees and providers and communicate pertinent information to relevant stakeholders

* Coordinate activities between our Department of Pharmacy, retail pharmacies, pharmacy residency program and the outpatient clinics

* Serve on a standing committee member on select committees as needed within UH Health System

  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
