Description

  • Organizes and conducts Occupational Therapy program to facilitate the rehabilitation of people with physical and or psychosocial disabilities.
  • The Occupational Therapist directs the patients participation in selected activities, treatments to restore, re enforce and correct pathology to promote and, or maintain good health.
  • Essential duties include, Performs evaluations and develops an effective treatment plan appropriate to a patients rehabilitation needs within the care continuum.
  • Performs effective treatment techniques in a competent manner.
  • Educates patient, family, caregiver in applicable health information, treatment and prevention. Initiates and participates in the discharge planning process.
  • Performs other department activities such as, but not limited to documentation, billing, support of department operations, etc.
  • Professional initiative, objectivity, good time management and interpersonal skills.
  • Must be physically, cognitively and perceptually able to provide patient treatment and able to assist in physical mobility of patients and equipment.
  • This position is in Home Care Services.
  • Monday-Friday with rotating weekends.
  • 100% traveling to the patients homes for visits.
  • This is the day shift.
  • 30 Visits per week to patients on Home Care Services.
  • One Saturday every 6 weeks.
  • The territory will be:
  • * Eastern Cuyahoga County primarily City of Cleveland but not limited to: and Downtown, Central, Southern Cuyahoga, Garfield, Shaker Hts., Euclid, Richmond Hts., Cleveland Hts., Mayfield Hts., Beachwood, Pepper Pike Lyndhurst, University Hts., South Euclid, Bedford, Solon, Maple Hts., Twinsburg, Hudson, Chagrin Falls, Macedonia, Northfield and Pennisula.
