Job Summary: Functions in an expanded role as a professional advanced practice provider, performing acts of medical diagnosis and prescription of medical therapeutics or corrective measures, in collaboration with a physician, performing duties in the outpatient and inpatient (as required/assigned) setting. The APP works with an interdisciplinary team of primary caregivers and administrative support staff to perform assigned duties while displaying the highest standards of ethical and moral conduct as well as acting in the best interest of Aultman Medical Group, supporting our mission, vision, and values to optimize the health and well-being of our patients. The APP will provide coverage at more than one of the Cardiovascular Consultants locations.

Primary Job Responsibilities:

The following is not an exhaustive list of responsibilities. This list of essential duties and responsibilities is intended to provide a representative summary of the major duties and responsibilities performed. Incumbents may be required to perform other job-related tasks other than those specifically presented in this job description.

  • Obtains and documents health history, history of chief complaint, and physical examination upon office visit, patient admission or physician consultation.
  • Manages patients in general cardiology, working in collaboration with RNs, MAs, receptionist, and physician
  • In collaboration with the physician, orders medical consultations, therapies, and/or interventions including, but not limited to, medications, diagnostic tests, respiratory care, nutrition, blood and blood products, fluid and electrolyte support. Implements evidence-based practice guidelines when indicated.
  • Collaborates with the physician to provide consultative services in the hospital, as well as manage care inpatient as needed
  • Provides a safe environment for patients.
  • Assesses needs of patient for plan of care.
  • Performs invasive and therapeutic diagnostic procedures based on the needs of the patient as determined by the physician and as approved and credentialed by the Medical Staff Office. Documents procedures, patient response and results in the patient's medical record.
  • Provides education and support to patients and families.
  • Facilitates communication between patient and family members and members of the health care team.
  • Under the direction of and in collaboration with the attending physician or designee, formalizes the discharge planning needs of selected patients.
  • Responds to changes in patient's condition by assessing the patient via history and physical exam appropriate to such change; contacts the physician to relay the findings and formulate necessary interventions; orders laboratory tests and other diagnostic studies if further data is necessary, and orders medication in accordance with the State of Ohio and Medical Staff By-Laws.
  • Serves as a clinical resource for nursing staff, allied health staff, residents, fellows, interns, and other patient caregivers.
  • Serves as a clinical preceptor for APP's and other healthcare professionals, including residents, fellows, and interns.
  • Initiates and/or collaborates in performing clinical research projects including retrospective and prospective study registries and the preparation and presentation of information.
  • Participates in quality of care improvement through the development and evaluation of patient care indicators and the systematic review of patient records and protocols to determine effectiveness in meeting established standards of care.
  • Promotes education in classroom, clinical setting and community.
  • Maintains professional development within the advanced practice role.
  • Follows the policies, standards, protocols, and procedures of Aultman Health Foundation
  • Participates in economical utilization of supplies.
  • Abides by the Code of Conduct at all times
  • Supports strategic organizational goals
  • Completes all Medical Records as required by Medical Staff bylaws, Rules and Regulations of Hospital
  • Creates and fosters a work environment that is consistent with the Mission, Vision and Values of Aultman Health Foundation
  • Comply with established record keeping, billing, and coding policies

Education/Training:

Registered Nurse or Physician Assistant in the State of Ohio (or eligible); Master's degree in nursing with successful completion of a graduate level APRN program or Physician Assistant program.

Scope of Practice:

Under the Standard Care Agreement in collaboration with the Cardiologists, the APP assists with the management of the heart failure patient. Medical diagnostics, therapeutics, and medical management functions will be carried out in accordance to guidelines established by the State of Ohio, Aultman Medical Staff Bylaws and hospital policies, procedures, and protocols.

Physical Requirements:

Sits, stands, and moves frequently throughout the work schedule. Lifting, bending, twisting, pushing, pulling, reaching, kneeling, and climbing stairs. Must have ability to handle all situations with emotional stability.

Working Conditions:

Subject to varying and unpredictable situations and frequent interruptions.

Additional Requirements:

  1. Continuing medical education (CME) as required

Knowledge, Skills, and Abilities:

Knowledge of and ability to apply professional primary care medicine principles, procedures, and techniques. Thorough knowledge of pharmacological agents used in patient treatment. Ability to effectively manage and direct medical staff support activities while providing quality patient care. Ability to receive detailed information and express and exchange ideas orally. Ability to perform under stress.

Possesses excellent written and oral communication, listening, and social skills. Ability to interact effectively with people of varied educational, socioeconomic, and ethnic backgrounds, skill levels, and value systems. Professionally performs in a tactful manner. Taps experience and uses independent judgment to plan and accomplish goals.
